Online access to the screening questionnaires

The cost for a single use of the LDSQ and associated result is £5.00. You can pay for this via our shop.

Please contact us for a price if you require multiple uses of the online LDSQ.

Once we have received payment, you will be emailed a password, which will allow you to access the online LDSQ. Please note that it can take between a few hours and a few days to receive payment, depending on the payment method used, so please allow some time for the password to reach you. Please check your junk mail, as well as your inbox for our email.

You will receive the results by email once you have completed the questionnaire. Please check your email junk folder in case the results are sent there.

Please note that the questions must be answered in one go. You can’t save and finish later.

If the results of the online questionnaire indicates that the child/young person or adult may have an intellectual disability and you have concerns about him/her, you may wish to contact your local services (for example, GP, paediatrician or intellectual disability service) to ask for advice. You can also visit the British Psychological Society website, which provides information about chartered applied psychologists in your local area in the UK who may be able to provide a private assessment of the person: https://www.bps.org.uk/public/find-psychologist

If you live in the North East of England or South East of Scotland and would like a private assessment carried out by one of our Chartered, Health and Care Professions Council registered Clinical Psychologists, you can contact us on enquiries@gcmrecords.co.uk

The CAIDS-Q and LDSQ have high levels of accuracy, however, they are less accurate under the following circumstances:

  • If the child is aged under 8 or if the young person is aged between 16 and 18 years.
  • If the person has an existing condition that may impact on the results of the screening questionnaire, such as a physical disability, specific learning difficulties, or Autism Spectrum Disorder.
  • If the person has English as a second language
  • If you had to guess the answers to any of the questions
